Formulaire pour vous inscrire ou inscrire une personnes aux services d'INCA. Nous avons créé ce formulaire afin de faciliter la procédure pour les professionnels de la santé ou les intervenants en déficience visuelle qui souhaitent référer un patient vers nos services.

Notez que toutes les personnes vivant avec une limitation visuelle et leurs proches sont admissibles à nos programmes (dès que leur vision commence à perturber leur vie quotidienne).  Nul besoin d'être légalement aveugle ou référé par un professionnel de la santé.  Pas besoin non plus d'être membre pour participer aux services d'INCA, il suffit de s'inscrire pour participer à la formation, au groupe ou à l'atelier qui vous intéresse.
